Fleck Foundation - Application Process

Please follow the application procedure in the order requested.
Agencies interested in applying for funding should submit Section I of the application to the Foundation.
The Foundation will determine our interest and advise the applicant on whether to submit a complete application.
Upon receipt of the complete application, we will arrange a site visit.
The application is to be completed in outline format.
Grantees will be required to sign a grant agreement.
A final report describing results and the utilization of the funds is required.

Applications are accepted on an ongoing basis.
Board meetings are held monthly.
Applications will be considered at the next board meeting if a complete review can be made.

Section I: Initial Request

Section II: Grant Description

Need for the grant and how was the need determined
How many people will be served and how will they benefit
Expected outcomes and the methods that will be used to measure them
Implementation timetable
List names, title, credentials, and length of service of the people providing leadership for the activities funded by grant request
Program staffing requirements
List of any collaborating organizations

Section III: Leadership

Key leadership
- List names, title, credentials, and length of employment of the key leadership of the organization
- List of Board of Directors including names, title, length of service on the board, and phone numbers
Staffing of Organization
- Number of full time and part time employees
- Number of volunteers and a summary of the role volunteers play in the organization
- Organization chart

Section IV: Financial Information

Detailed program budget
List of other financial support for this program
Plans to sustain funding
Audited financial statements, including notes, for the last two or, preferably, three years.
The most recent financial reports.
